Is the name on your documents different from the one on file with your insurance company or in government records?
The way your name is spelled on your residence permit may differ from the way it appears in the records of certain Czech authorities. This is due to your civil registry identity.
A civil registry identity is created if you have been issued a Czech civil registry document, such as a birth certificate or marriage certificate. In these documents, your name may be recorded using the Czech transliteration, which can differ from the name shown on your residence permit. Your residence permit must be issued in the same name as your passport.
In the Population Register, your civil registry identity always takes precedence. As a result, many government authorities and other institutions will identify and register you under that name. When dealing with these bodies, we recommend that you inform the staff of your civil registry identity in advance.
If you hold a biometric residence card or a permanent residence permit card, you can have your civil registry identity entered in the remarks section of the card.
